A $10 Billion Installment of a Five-Year Program
The Rural Health Transformation Program, born from the One Big Beautiful Bill Act last summer, aims to offset the devastating impact of Medicaid spending cuts on rural communities. With a total budget of $50 billion over five years, this first installment represents just 20% of the promised funds.

A Tight Deadline Looms
States have eight months to submit revised budgets, begin spending, and demonstrate that these dollars are being used effectively. Federal officials will review state progress in late summer, with funding levels for 2027 announced by October.

The Stakes Are High
With allocations ranging from $147 million for New Jersey to $281 million for Texas, the pressure is on states to prove their worth. Failure to meet these expectations could mean a significant reduction in funding for next year.
